Piper Alpha videoSEMS II - like most process safety regulations - has a Stop Work provision. But offshore platforms have a near-military structure where orders and given and obeyed.

If the Claymore employee had disobeyed his OIM it is likely that many lives would have been saved. But he chose not to act without explicit orders.

Quantification of risk

The term Prestartup Safety Review is used by OSHA in its Process Safety Management standard. BSEE (Bureau of Safety and Environmental Enforcement) uses the term Prestartup Review. A more comprehensive term is Operational Readiness. This term includes concepts such as "Care, Custody and Control", i.e., ensuring that the facility as built exactly matches what the designers intended.

Regardless of the term that is used, this ebook provides useful guidance on this important topic.

The PSM Report

For the last few months we have been publishing The SEMS Report. It provides a mix of news items and technical discussions to do with the Safety and Environmental Management Systems rule for offshore operations on the Outer Continental Shelf of the United States.

The response to The SEMS Report has been good so we have decided to start a new publication, The PSM Report. It will cover process safety management issues for onshore and offshore facilities around the world.
